Intake Forms & Questionnaires
The following intake forms are required upon arrival to your first visit. Forms are available for download and completion prior to the time of evaluation. Please complete the Medical History, Financial Policy, Privacy Practices, Appointment Reminder Consent. Preferred option is to complete the initial paperwork online through our patient portal. Forms will also be available in the clinic, please arrive 15-20 minutes before your scheduled appointment to allow ample time for check-in and paperwork. We will confirm your contact information, explain your insurance benefits, and collect photo ID, insurance card, and perscription/referral at the time of your first appointment.
