Commonly Asked Questions Prior to Your First Visit

Does Forever Fit Physical Therapy & Wellness accept my insurance?

Yes, Forever Fit Physical Therapy & Wellness LLC accepts payment from many major insurances We will be glad to help you determine your insurance coverage prior to coming for your first visit. More comprehensive insurance information can be found on our Insurance FAQ page.

What should I expect the first visit?
The first visit will consist primarily of a comprehensive examination. This will include questions about your injury and general medical history, observations by the physical therapist, tests of strength, range of motion, functional tasks, and special tests designed for diagnosing the problem. Together the results of this exam will help the physical therapist design your unique treatment program. Typically, you will receive a brief treatment after the exam. The total first visit lasts approximately 45-60 minutes.
What should I wear?
The physical therapist will need to be able to view the injured body part(s). For your comfort it is recommended that you wear loose fitting clothing that will allow unrestricted movement and allow access to the body region (eg. Shorts for knee injuries, tank tops for shoulder injuries etc).
What should I bring for my first visit?
  • Insurance card
  • Photo ID
  • A list of medications you are currently taking
  • Your prescription/referral if you have one
  • Any relevant test results
Do I need a referral or prescription from my physician to see a physical therapist?

The state of Maryland has direct access for physical therapists, meaning you do NOT need a referral to see a physical therapist. However, if you have Medicare or an HMO insurance plan, you may be required to have a referral for your insurance company to pay for treatment. Call 301-421-1125 and we will be happy to help you determine your plan’s requirements.

Should I have X-rays or MRIs before coming to see a physical therapist?
No. In the absence of serious trauma, an MRI is rarely needed before starting physical therapy. Our highly trained physical therapists are educated to screen patients to determine if further imaging or diagnosis is necessary during their examination. For most injuries, physical therapy is not only SAFE but recommended before ordering expensive tests, as most conditions found on MRIs will still require physical therapy before any invasive procedures are considered. If you have had any of these tests and have the results, please bring them to share with your physical therapist.
How do I request for medical records?

If you or a documented 3rd party is in need of your medical records, you will need to print and complete the medical request form. Please complete the form and return to us via email at [email protected] or fax at 301-500-2175. Once the form is returned along with payment, you will recieve your records within 5 business days.
